1/13/2024 EnchantaVaughn Approved 1.Behavior: Josiah arrived to session appearing to be in a calm mood by greeting QMHP-C and appeared to be well-groomed. Josiah was struggling with focusing to the instructions as evidenced by Josiah playing on his phone and influenced by his surroundings. Josiah was having difficulties with coping as evidenced by Josiah refusing to be supportive with helping others and being confidence in his choices. Josiah was refusing to be positive with making positive choices and being accountable for his actions with behaviors lasting about 10 minutes in duration. 2. Behavior: Josiah started to shut down as QMHP-C implied his behaviors. Josiah was refuses to express the situation and continued to respond nothing. Josiah was shutting down as QMHP-C asked questions. Josiah continues to struggle with engaging appropriate and showing positive self-esteem. Josiah refuses to be support by others and continued to shut down when getting assisted. Josiah struggled with having confidence when ask to complete a task as evidenced by Josiah stating "I Can’t" and walked away from the activities at which mother reports his behaviors last abut 10 minutes in duration. 1.Intervention: QMHP-C infuenced Josiah be accountable for his actions and identify his self-images. QMHP-C provided role playing activities to help the interview himself. QMHP-C help Josiah with defining self-images and how to create posituve images about himself. QMHP-C provded awareness wheel for Josiah to help identify himself. QMHP-C assisted Josiah with self-help guide to overcoming symptoms of stress, anxiety and low.QMHP-C provided several worksheet to help Josiah with identify himself and understand the characterist of himself and see if his friends to identify himself. 2. Intervention: QMHP-C addressed Josiah’s behavioral issues on self-images. QMHP-C recommended Josiah to express himself and refrain for being nonchalant. QMHP-C recognized Josiah’s positive mood and behaviors and encouraged him to maintain his current mood with self-checks and positive self-talk. QMHP-C encouraged Josiah to utilize his ability of being honest and taking responsibility. QMHP-C discussed with Josiah on the ability to manage behaviors according to the classroom expectations with effective self-monitoring. 1.Response: Jashim was receptive to QMHP-C's suggestions for today's session. Josiah greeted QMHP-C appropriately by using appropriate tone and being respectful by showing eye contact. Josiah responded aggressively when asked to put his phone way. Josiah was struggling with completing his activities as evidenced by not responding about himself. Josiah was receptive after several redirections as evidenced by identifying his emotions and be aware of his self-awareness by making positive choices. 2. Response: Josiah was receptive of feedback given and verbalized an understanding of advice. Josiah acknowledged feedback and was able to express insight and spoke clearly. Josiah was able to successfully follow directions and implemented skills without difficulties.
